Making a Referral

Making great occupational therapy referrals - Advice from the Royal College of Occupational Therapy

Referral form for children living in Conwy or Denbighshire

Age norms for functional activities - Please only refer the child if their functional performance differs significantly from these norms for eating, dressing, toileting, self-care and bathing.

Making a referral
We very much welcome appropriate and timely referrals for children whose difficulties in achieving everyday activities are:

  • below expectations of the child's developmental age.
  • significantly impacting on the child's function.
  • important to the child and family to improve.
  • significantly impacting on their learning needs.
  • important at this particular time.

Some needs of children referred could be met by implementation of strategies found on our advice section of this site. Please consider trialling these before making a referral.

Sensory Processing Referrals  - This team cannot offer sensory assessments as a way of explaining a child's behavioural and social issues. We will do our best to signpost the referrer to suitable alternative pathways if they exist.

We consider referrals from:

  • Paediatricians, GP’s, school nurses and other health professionals
  • Social services and children's teams
  • Educational psychologists and additional learning needs coordinators (ALNCO's)
  • Parents/ carers can only self-refer if the child has previously been known to the service.

To discuss any referrals please contact us on 03000 855 962

How to refer

The referral should be filled out in full by a referrer who has first-hand knowledge of the child’s difficulties.

We offer all children the opportunity for an appointment within 14 weeks of the receipt of the referral. If you think that the wait may have exceeded this or if at any time you need to check the status of the child on the waiting list please phone.

Once the referral has been received we will make a decision whether to offer an assessment based on the referral information provided. If the referral is accepted then the child will be put on the waiting list and allocated to a specific therapist.

On occasions we may feel that the child's difficulties as presented on the referral are not appropriate for our service and their needs may be best met by another service. We will inform the referrer who will be responsible for making the referral to the appropriate service.
