Making a Referral

To be eligible for the service the child or young person must:

  • Be under the age of 18 (or still in full time education)
  • Be over the age of 5 for assessment of general fine and/or gross motor skills
  • Live or attend school in Gwynedd and Mon and
  • Be experiencing significant difficulties with everyday tasks which are below expectations of the child's developmental age.
  • They must significantly impacting on the child's function.
  • important to the child and family to improve.
  • significantly impacting on their learning needs.
  • important at this particular time.

Sensory Processing Referrals  - This team cannot offer sensory assessments as a way of explaining a child's behavioural and social issues. We will do our best to signpost the referrer to suitable alternative pathways if they exist. More information and advice can be found on our sensory processing pages 

How to refer

The referrer must have obtained permission from the parents/ guardians to make the referral. We will process this request in good faith that you have the parent’s permission.

We usually offer all children the opportunity for an appointment on the receipt of the referral. 

Once the referral has been received, we will make a decision whether to offer an assessment based on the information provided. If the referral is accepted then the child will be put on the waiting list and allocated to a specific therapist.

On occasions we may feel that the child's difficulties, as presented on the referral, are not appropriate for our service and their needs may be best met by another service. We will inform the referrer who will be responsible for making the referral to the appropriate service.

 At present, we consider referrals from:

  • Paediatricians, GPs, school nurses and other health professionals
  • Social services
  • Educational psychologists and additional learning needs coordinators (ALNCOs)

 We do not accept new referrals from parents, but we can accept referrals from parents if they have been known to the service in the past.
