Children's Occupational Therapy Referrals

 

How to Refer

The Occupational Therapy or Developmental Co-ordination Disorder (DCD) Therapy referral forms should be filled out in full by the referrer who has first-hand knowledge of the child’s difficulties.

As a referrer you have an obligation to obtain the parents/ guardians permission to make this referral to our service. We will receive and process this request in good faith that you have met this obligation.

Children’s Occupational Therapy Referrals:-

This specialist Occupational Therapy Service provides assessment and therapeutic intervention for a wide range of children who have complex physical or developmental conditions significantly affecting their ability to achieve their optimum level of independence and participation in childhood occupations.

Access to Service

In order to be eligible for the service the child must:-

  • Live or attend a school in BCUHB East catchment area.
  • Be in full time education / under 18 years or age.
  • Be experiencing significant difficulties with their functional performance in areas of occupation – self-care; productivity/activity and leisure/play.
  • Require postural positioning/equipment to maximize ADL’s.
  • Require environmental adaptions within education.

Referrals will be considered for acceptance to the service on receipt of a completed Children’s Occupational Therapy Referral

Referrals are accepted from Paediatricians and their team members, GP’s, Allied Health Professionals, Educational Psychologists and nursing services.

We offer a telephone advice/support on Monday Mornings and support via BCU email for professionals to discuss referrals. (Active and pending/potential)

There is now an Occupational Therapist working within the Neurodevelopmental Service in CAMHS based within the Wrexham Child Health Centre :- Telephone 01978 726013.

Referral Triage

Once received the referral will be triaged and a decision made whether to accept the referral based on the information provided on the form. 

We will send out parent and or teacher questionnaires to be completed to get a better understanding of the child’s needs from Occupational Therapy. These should be returned within 2 weeks of the date on the letter.

Once we receive the questionnaires back, the Occupational Therapists will re-evaluate all the information provided and there are a number of pathways the Occupational Therapist will decide as best for your child :-

  • Telephone Consultation
  • Virtual Consultant
  • Face to Face Consultation
  • School Visit
  • Life Skills Consultation

On occasions we feel that the child’s difficulties as presented on the referral are not appropriate for our service and their needs may be best met by another service.  We will write the referrer and copy to parents to make the appropriate referral.
