Our Service

Our nurses co-ordinate the completion of initial health assessments by a paediatrician when a child first comes into care, and will review health assessments until the child turns 18 years or leaves care.

Initial health assessments are completed within the first few weeks of the child coming into care, and review health assessments are completed every 6 months until the child is 5 years then every 12 months until the young person turns 18 years or leaves care.

Our nurses offer information and support around a range of issues such as; stopping smoking, healthy eating/healthy weight, healthy relationships, sexual health advice, drugs and alcohol awareness.

They can refer you onto other services, with your consent, for example;

Our nurses work collaboratively with a range of other professionals and services such as safeguarding teams, local authority, health services and voluntary services to ensure all aspects of a looked after child’s needs are met.

How do I access the service?

The looked after children team are available primarily Monday to Friday, 9am to 5pm (excluding bank holidays), and are available during the school holidays.
