Anglesey

Specialist Children’s service is the Integrated Team for Disabled Children, working with children and young people aged 0 - 18 on Anglesey. It provides specialist support for:

  • children with a developmental delay or disorder
  • children with a learning disability
  • disabled children
  • children with an illness.
  • Children with significant physical disabilities
  • Children with significant Sensory impairments.

Specialist Children’s Services is a partnership between the Betsi Cadwaladr University Health Board and Anglesey County Council’s Social Services Department.

Specialist Children’s Services work with disabled children who have: -

  • A disability, significant developmental delay or a learning disability  where mainstream services are unable to meet their specialist need

SCS can offer –

  • a single point of contact for the Team
  • advice and information
  • an Initial Assessment by a Duty Officer
  • professional input to meet the child’s needs
  • multi-agency review of the child’s needs
  • a right to an assessment of the carer’s needs
  • short breaks.

Who can refer a child to SCS–

  • the child or young person
  • parent / carer
  • professionals (with family consent) 

The Integrated Team includes  Duty Customer Care officer,  Children’s Community  Nurses (Learning disabilities),Clinical Psychologists, Social Workers, Support Services, Transition co-ordinator and Occupational Therapist (Adaptations)

The Team’s aim is to: 

The nurses and nursing assistants work in partnership with the parents and carers, other team members, health professionals, social services, education authority and relevant voluntary agencies to help the child develop his or her maximum potential.

The service is provided within the child’s environment, e.g. within the home, play group, school, leisure facilities, in Specialist Children’s Service centres and other health premises.

Eligibility:

  • Children from birth to 18 years.
  • Children with 2 or more severe delays in development one of which must be cognition (pre-school)
  • Children with learning disability (School age+)

Referral process:

Following an assessment and further discussions, we are able to offer individually tailored programmes and Care Plans to assist in the Child’s or Young Person’s development in various areas such as: Play and Development, Self Help Skills such as Toileting, Washing, Dressing and Feeding, Bedtime/Sleeping Difficulties, Behavioural issues and the generalising of these skills  to all settings as appropriate. We also offer Workshops and Group Activities as we feel appropriate.

We aim to empower Young People, and their Families /Carers in the development and maintenance of new and developing skills by providing the information and knowledge they need in order to learn and maintain these skills. We would provide written information in the form of Programmes and Care Plans for the individuals concerned to follow and support them through this process.
