If your child is under the school age learning disability team and still currently open and active to our team when they are 17 we would recommend (if families/carers) feel necessary, a referral to the adult learning disability team. we would refer from 17 ½ years old and be able to give the adult team a thorough handover regarding your child and the work we have been completing. If your child is not open to our team when they are about to turn 18years old, you can contact the Wrexham adult's community learning disability team who will be able to support you with a referral and answer any questions you may have.
