Wrexham Childrens development and Learning disability team aims to make sure that all children who meet our criteria have a right to an ordinary pattern of life within the community, to be treated as individuals with access to additional specialist help to ensure their maximum health and development.

The team is split into two areas, pre-school development team and the school age learning disability team. Our team is made up of community nurses, specialist nursery nurses, admin and clinical psychologists who will then work predominantly with families and interlinking into schools, GPs, speech and language, social services, paediatricians and many more.

Pre-school eligibility:

Children aged 0-5 who are presenting with development delay in more than 2 areas and/or social and communication difficulties or concerns regarding possible ASD (autism spectrum disorder).

School age eligibility:

Children aged 5-18 who have a learning disability (significant global development delay) and an associated health need.

Referral process (Wrexham): Complete MAP (multi agency panel) referral with parental signature/consent. Referral can be completed by professionals or parents. If referral is accepted you will then go on to the waiting list for a CAPA (choice and partnership appointment) to find out more about your child’s needs. Once allocated a keyworker, we can then offer advice and guidance around the areas of need e.g. behavioural interventions, developing good sleep hygiene, guidance for toilet training etc. Interventions are individualised to each child and the family's needs based on the outcome of CAPA. We can also refer onto different professionals for further specialist support if required.
