GP In-Reach

Who are we?​

We are a small team of children’s mental health practitioners working with your local GP Practices across Flintshire and Wrexham. We are an early intervention and prevention service offering mental health and wellbeing support to children and young people.​

How to access our services? ​

If you are struggling with your mental health and wellbeing, making an appointment with your GP can be a great first step in accessing support. ​

Depending on your needs, your GP may refer you to see us at your surgery.

Is this service for me?​

We see children and young people with mild mental health struggles such as issues with anxiety, low mood and stress.​

Our service is available for anyone over the age of 7 years and under the age of 18 years. For more information about support available if you are under 7 years please see : (link to early years pages)​

Services we offer? 

Professional Consultation​

Sometimes it can be difficult to get the right help at the right time and you don’t want to have to wait or speak to someone new about your struggles. To help you with this, we can advise your GP to help you access the support you need without another appointment. This what we call professional consultation. 

Appointment​

If you are referred to us, you may be offered a face to face appointment or a telephone appointment. This appointment will help us get to know you and to give us an idea of your struggles. Once we know what matters to you, we can work together to develop an agreed plan of support and may offer you advice, signposting to other services available or up to four sessions.​

We will make contact with you or your parent to arrange a date and time for either a telephone or face to face appointment to see us at your GP practice. ​

When you arrive at your GP Practice, let the receptionist know you have arrived and the name of the person you have an appointment with. Take a seat, the receptionist will let us know you have arrived and we will call you in for your appointment. 

Our team are made up of:​​

  • Mental Health Nurses​
  • Mental Health Social Workers​
  • Learning disability nurses​
  • Occupational Therapists​
  • Psychologist ​
  • Team Secretary​
  • Or other professionals

What is advice?​

Advice can be helpful to support you in making decisions that best help your needs. Some of this advice may be self-help techniques or what we call Psycho-education which is supporting you to understand the facts about your feelings and emotions. This advice may also be helpful for your family, friends and carers in supporting you. 

What is signposting/onward referrals?​

There are a lot of other services that work alongside us who we know can help you to improve your wellbeing. Signposting is providing information about these services and what they can do for you to contact them directly. Onward referrals are where we can support you to access these services. ​

These others services are based in your local communities and schools and can offer counselling, support groups, peer support, parent support, play therapy, getting out and about and many other things. ​

What are the sessions?​

Sessions are up to an 1 hour long and are offered every few weeks up to four times. These sessions may cover coping strategies such as managing anxiety, sleep hygiene, challenging negative thoughts, 5 ways to wellbeing strategies, relaxation, mindfulness and/or guided meditation. We use different resources including self-help guides, videos, iPads, art and crafts and things you can take away to do in-between sessions. ​

Can I bring someone with me? ​

You can choose to have your family or someone to support you in your appointments if you feel this would help you. We know talking to someone new can be scary, so you can choose to bring something to your appointment to help you feel comfortable such a fidget toy or blanket. You are also always given the option to speak to us while your parents or carers waits in the waiting room.
