EPP Cymru

EPP Cymru (Education Programmes for Patients) provide and deliver a range of free health and wellbeing courses aimed at adults who are living with or caring for someone with a long term health condition(s).

EPP Cymru courses will help you to:

  • Recognise and manage symptoms
  • Develop more effective relationships with those supporting you
  • Work in partnership with healthcare staff
  • Develop new skills and knowledge to improve your quality of life
  • Have more confidence

The courses are run by trained tutors with personal experience of a health condition or a caring role.  The programmes are scripted and frequent assessments ensure consistent quality in delivery.

Courses available

Health and Wellbeing - aims to help people who live with a long term physical or mental health condition to maintain and improve their quality of life through self-management.  The course is available for anyone who is 18 years old and over and lives with symptoms for 6 months or more.  

 Subjects covered include: pain and fatigue management, improving sleep, dealing with low mood and difficult emotions, preventing falls, better breathing, medication usage, physical activity, healthy eating, action planning, problem solving and decision making.

Introduction to Health and Wellbeing  - aims to help people who live with a long term physical or mental health condition to maintain and improve their quality of life through self-management.  The course is available to anyone who is 18 years old and over and lives with symptoms for 6 months or more and unpaid carers. 

Subjects covered include: positive thinking, managing daily activities, communication, relaxation, medication, physical activity, healthy eating, action planning and problem solving. 

Course overview

Diabetes Self Management - aims to help people who live with type 2 diabetes to maintain and improve their quality of life through self-management.  The course is available for anyone who is 18 years old and over and lives with type 2 diabetes.  

Subjects covered include: preventing low blood glucose, preventing complications, monitoring, healthy eating, physical activity, foot care, mediation usage, managing sick days, dealing with stress, dealing low mood and difficult emotions, action planning, problem solving and decision making.

COPD Self Management for Life  - aims  to help people who live with breathlessness to maintain and improve their quality of life through self-management on a daily basis and particularly during inevitable exacerbations.  The course is available for anyone who is 18 years old and over and lives with COPD. 

 Subjects covered include:  understanding COPD, breathing techniques, managing breathlessness, recognising and managing setbacks and exacerbations, fatigue management, planning to stay well, pacing, improving sleep, physical activity, introduction to mindfulness, goal setting, action planning and problem solving. 

Course overview

Cancer: Thriving and Surviving - aims to help people who have been affected by cancer to maintain and improve their quality of life through self-management.  The course is available to anyone who is 18 years old and over and living with, receiving treatment for or in remission from cancer. 

Subjects covered include: pain and fatigue management, living with uncertainty, body changes, dealing with difficult emotions and low mood, improving sleep, making treatment decisions, physical activity, healthy eating, action planning, problem solving and decision making.

Chronic Pain Management - aims to help people who live with chronic pain to maintain and improve their quality of life through self-management.  The course is available for anyone who is 18 years old and over and lives with chronic pain  

 Subjects covered include: understanding what chronic pain is, pacing and planning, improving sleep, fatigue management, weight management, dealing with difficult emotions and low mood, better breathing, stress management, physical activity, healthy eating, action planning, problem solving and decision making.

Introduction to Chronic Pain Self-Management - aims to help people who live with chronic pain to maintain and improve their quality of life through self-management.  The course is available for anyone who is 18 years old and over and lives with chronic pain

Caring for Me and You - aims to help people who care for someone with a long term health condition to maintain and improve their quality of life through self-management.  The course is available for anyone who is 18 years old and over and undertakes an unpaid caring role. 

 Subjects covered include:  staying healthy, improving fatigue, improving sleep, getting help, challenging behaviours, dealing with difficult thoughts and emotions, future planning and legal issues, relaxation, action planning, problem solving and decision making.

Long Covid Self Management - aims to help people who have had Coronavirus and are experiencing long Covid / post viral symptoms to maintain and improve their quality of life through self-management.  The course is available for anyone who is 18 years old and over and lives with Long Covid. 

Subjects covered include: pain and fatigue management, improving sleep, dealing with low mood and difficult emotions, preventing falls, better breathing, medication usage, physical activity, healthy eating, action planning, problem solving and decision making.

Living Well while Working - aims to help people who work while living with a long term health condition to maintain and improve their quality of life through self-management.  The course is available to anyone who is 18 years old and over, in work and living with symptoms that affects their lives for 6 months or more.  

Subjects covered include:  pain and fatigue management, stress management, improving sleep, time management, working toward balance, dealing with difficult emotions and low mood, physical activity, healthy eating, weight management, goal setting, action planning, problem solving and decision making.
