Subject Access Request (SAR)

A subject access request is a request for information made by or on behalf of an individual under the Data Protection Act to obtain personal information about them. The request can be made verbally or in writing and under legislation the Health Board have 28 calendar days to respond. 

If you wish to make a SAR request it would be helpful if you could fill in the SAR form.

Guidance on completing the form:

Be specific – help us identify the exact information you require. Please provide as much information possible on what information you require including dates.

Provide proof of your identity – for example, a passport, driver’s license, or birth certificate, together with other relevant information which can confirm your address, e.g. a utility bill;

Provide proof of relationship – if the information is regarding another person please provide proof of relationship and any necessary consent. 

Provide your contact details – your correspondence address and telephone number should you need to be contacted to discuss your request further.
