Making a Referral

Advice to help make an appropriate referral. Feel free to print off as a poster and share around your school/workplace.

Guidance on Age norms for functional activities - Please only refer the child if their functional performance differs significantly from these norms for eating, dressing, toileting, self-care and bathing. 

We consider referrals from:

  • Paediatricians, GP’s, school nurses and other health professionals
  • Social services and children's teams
  • Educational psychologists and additional learning needs coordinators (ALNCO's)
  • Parents/ carers can only self-refer if the child has previously been known to the service.

The referral should be filled out in full by a referrer who has first-hand knowledge of the child’s difficulties.

Referral Criteria: 

All children must be: 

Children with Physical disabilities:

Typically; Cerebral palsy, hemiplegia, trauma, muscular dystrophy, spina bifida, hand deformities. 

Can access our service from any age.

    Children with Co-ordination difficulties:

    Typically – Dyspraxia, ASD, ADHD, Developmental delay

    • Under 5 year olds

    Access is through a universal provision, ie advice through website and training of nursery staff. Development of fine motor skills for under 5’s and activities for under 5’s that help develop fine motor skills are readily available over the internet. Dyspraxia will not be typically considered for diagnosis until after 5 years old.

    • 5-19 years old

    Can access our service if experiencing significant and persistent problems with performance in everyday tasks and motor based school performance tasks. 

    Sensory Processing Referrals  - This team cannot offer sensory assessments as a way of explaining a child's behavioural and social issues. We will do our best to signpost the referrer to suitable alternative pathways if they exist. More information and advice can be found on our sensory processing pages
