Derwen Integrated Services
Derwen, the Integrated Team for Disabled Children, works with children and young people aged 0 - 18 in Gwynedd. The Team provides information, assessment, intervention and support for disabled children and young people with continuing needs as a result of disabilities or illness. The Team aims to support families, carers and the wider community in order to promote the health and welfare of disabled children and young people in Gwynedd
Derwen can offer –
- a single point of contact for the Team
- advice and information
- an assessment – dependant on need
- a care and support plan
- Regular monitoring and review
- a right to an assessment of the carer’s needs
Who can refer a child to Derwen –
- the child or young person
- parent / carer
- professionals (with family consent)
Derwen is a partnership between the Betsi Cadwaladr University Health Board and Gwynedd Council’s Children and Supporting Families Department. The Integrated Team includes Community Nurses (Child Development),Clinical Psychologists, Social Workers, Support Services Officers,Occupational Therapist (Adaptations), Customer Care and Duty Officer, Family First and Family Support Officers. (Also works closely with Community Paediatricians, Educational Representatives, Therapists, the Neurodevelopmental services, Health Visitors and other relevant professions.
We aim to work with the child/young person and their families in a holistic manner, to develop person-planned approaches. They also provide evidence-based practice through:
- Undertaking Initial Nursing Assessment/Developmental Assessments.
- Functional behaviour Assessment – BBAT.
- Formulation of behaviour programmes based on Functional Analysis (FA)
- Participate in Diagnostic Assessments such as ASD either formal if trained or informal as a member of MDT with knowledge of the child.
- Producing, maintaining and monitoring care plans – both standard and those required under statutory responsibilities.
- Parenting advice on management of child with LD – common problems at various developmental levels.
- Sleep – Sleep Hygiene, deal with common sleep problems.
- Toileting advice, programmes.
- Communication – social stories, picture scheduling etc.
- Basic skills development work (Daily Living Skills, personal care etc.)
- Emotional Regulation work
- Sexuality and relationships work – appropriate sexual /social behaviour public/private etc.
- Growing up and Keeping safe work.
- Personal hygiene work
- Puberty work
