Information Requests

How to obtain information from the Health Board

As a public authority Betsi Cadwaladr University Local Health Board provides its patients, staff and the general public with the right to access a range of information.  Please follow the links below to request the information you require in relation to:

  • Freedom of Information Act 2000 (FoIA) - a range of business information regarding the Health Board: submit a request
  • Environmental Information Regulations 2004 (EIR) - information around environmental issues regarding the Health Board: submit a request
  • Data protection legislation (SAR) - right to access personal information other than health records by making a ‘Subject Access Request’: submit a request
  • Data protection legislation (A2HR) - right to access personal information within health records by making a ‘Access to Health Records request’: Access to Health Records 
  • Access to Health Records Act 1990 (A2HR) - copies of a health record for a deceased individual: Access to Health Records 
  • Data protection legislation - individuals rights such as rectification, restriction of processing etc: submit a request
