Family Wellbeing

The service aims to empower children, young people and their families to identify and build upon their existing strengths by supporting them to explore a range of potential ways to promote their wellbeing.

 

Who will you see?

First you will see the GP and they will refer you to a Family Wellbeing Practitioner (FWP) in your local surgery. The FWP is a Senior Child and Adolescent Mental Health Practitioner.​

The Referral process

​The FWP only accepts referrals from the GP Practice and other practices in the area. Once we have the referral, we aim to contact you within 14 days, the initial contact will be via telephone. We can offer up to four sessions via telephone, face-to-face or a meeting at the child’s school.

 

The Family Wellbeing Practitioner can identify the following:

  • What is the problem?
  • How they can help?
  • What you can do to help yourself?
  • And where you can get the help and support?

 

How can this service help me?

During the initial discussion, if face to face is needed then we will make that appointment over the phone with you. Appointments will always be held in your GP surgery, in a doctor’s room. You don’t need to bring anything to the appointments, Report to the main reception and wait in the surgery waiting room.

Please bring anything with you that could make you feel more comfortable whilst waiting and during the appointment.

What do we do?

We can support you with Emotional wellbeing issues such as anxiety and low mood.

The service can give you somebody with the knowledge and experience to listen to you as a family and help to make sense of the difficulties you are having and offer guidance and advice on how you can help yourself, and share services out there which can offer more help.

The Family Wellbeing Practitioner can also refer you to CAMHS.
