The Health Board’s Community Children’s team for the Central area and Ty Gobaith have been recognised for their outstanding collaborative work, receiving the Partnership Award at this year’s BCUHB Staff Achievement Awards, held at Venue Cymru, Llandudno.
The partnership between the team and Ty Gobaith has been instrumental in enhancing the care and support of children with complex and life-limiting conditions across the community. By working closely together—sharing patient information, conducting joint visits, and coordinating care during hospital admissions—they ensure that the needs of children and their families are prioritised through excellent communication and seamless teamwork.
This collaboration allows the teams to provide acute community support when care needs escalate, aiming to keep children at home and prevent hospital admissions wherever possible. By engaging with families early and offering timely support, the partnership has reduced crisis episodes and ensured that essential referrals, symptom care planning are handled promptly and effectively.
Integrating Ty Gobaith’s specialist nurse within the children’s team has also created opportunities for enhanced training, knowledge sharing, and skill development for both staff and families, particularly in paediatric end-of-life care. In addition, the partnership has improved access to counselling, social work, and family support services, ensuring holistic care for children and their families.
The success of the collaboration in the central area of the Health Board has been so impactful that plans are now underway to replicate the model in the west of the health board.
The Partnership Award highlights the power of collaboration, demonstrating that when organisations unite with a shared purpose, they can achieve exceptional outcomes for the children and families they serve.
