The relevant Occupational Therapy referral forms should be filled out in full by the referrer who has first-hand knowledge of the child’s difficulties.
As a referrer you have an obligation to obtain the parents/ guardians permission to make this referral to our service. We will receive and process this request in good faith that you have met this obligation.
Children’s Occupational Therapy Service
In order to be eligible for the service the child must: -
· Live or attend a school in Wrexham or Flintshire.
· Be in full time education / under 18 years or age /relevant ALN provision
· Be experiencing significant difficulties with their functional performance in areas of occupation – self-care; productivity/activity and leisure/play.
Referrals are accepted from Paediatricians and their team members, GP’s, Allied Health Professionals, Educational Psychologists and nursing services.
Referrals will be considered for acceptance to the service on receipt of a completed Children’s Occupational Therapy Referral
We offer a telephone advice/support on Monday Mornings and support via BCU email for professionals to discuss referrals. (Active and pending/potential)
