The Community Paediatric Service is a secondary specialist care service that supports children (up to 18 years of age, sometimes a little longer if they are in a special educational setting) where there are concerns with their development, disability and other long term health conditions.
We have a role in co-ordinating children’s health care outside of the hospital setting in order to give them the best start and support them in reaching their potential. We are community based and see children in clinics and school settings.
Alongside working with children, young people and their families, we also work very closely with other services within health, including therapies, school nursing & health visitors. We also work in partnership with other agencies, including schools, our education authorities, children’s hospices, children’s services across North Wales.
We work within the Health Board and across agencies to develop and implement policies to improve the health of all children across North Wales – particularly in the areas of children’s safeguarding, children who are in care’ vaccination and in partnership with Public Health Wales.
The Community Paediatric team aims to provide a high quality, specialised service for children with significant developmental impairment, disability and complex health needs.
Children can to be referred by a professional from the health service, education service (including their school) or children’s services. Most of our referrals are received from the GP or our health visitors and school nurses.
We do not accept referrals from young people or their families directly.
