Public Services Ombudsman (Wales) Act 2005

Notice pursuant to s.17(3) of the above Act.

The Public Services Ombudsman for Wales (The Ombudsman) has investigated a complaint and found maladministration/ service failure by Gwynedd Council, Betsi Cadwaladr University Health Board and Cartrefi Cymru (“CC”), a registered domiciliary care provider, and has sent a report on the results of his investigation to all three bodies. The complaint related to:

a) The care given to Mr N by CC
b) Failings in communication between the Council, the Health Board and CC, resulting in CC not receiving comprehensive documentation/ risk assessments/ care plans for Mr N.

The report will be available for a period of 3 weeks from 06/02/2020 and anyone who wishes may take a copy of this report or make extracts therefrom. Photocopies of the report or parts thereof will be provided on payment of 10 pence per sheet.
